Consent Form
This form is used to obtain consent from parents, guardians, or adult learners regarding the attendance of physical classes at Educational Coaching Center during the period in which online classes have been extended by the Ministry due to current regional safety concerns.
While ECC continues to support and encourage online learning, students who wish to attend optional physical classes at our campus may do so with proper consent. This form confirms that the parent/guardian or adult learner acknowledges the current circumstances, understands the associated risks, and agrees to follow all safety guidelines implemented by ECC and the relevant UAE authorities.
Submitting this form allows ECC to properly document consent and arrange the student’s preferred mode of attendance (physical or online).
